Incident/Accident Investigation
The Dade Moeller's safety staff includes subject matter experts in industrial hygiene, safety engineering, health physics, process safety, hazard communication, consequence assessment, radiation dose reconstruction, firearm/range safety, general aviation, and environmental protection. Our staff have been involved in a number of high-profile DOE investigations of waste tank vapor exposures, beryllium hazards, radioactive material inhalation, unplanned external radiation exposures, and safety integration into work control processes.
We have led or participated in a number of accident investigations including Type B and Type C accident investigations and personnel overexposure investigations, reviewed personnel contaminations, investigated lost radioactive sources, and provided radiological and hazardous material emergency response. We offer staff members trained in root cause analysis and corrective action development, and we provide an independent and disciplined approach to accident investigations.
